ENTERTAINMENT CONTRACT

This contract is for the professional disc jockey services exclusively featuring Artist
“DJ Lo-Skee” also known as Khalid Ali for the following (DATE)___/___/____
Starting at (TIME) ___:___(AM/PM) Ending (TIME) ___:____(AM/PM) (DATE)___/___/
______ between the aforementioned and undersigned Purchaser -(CLIENT) and
L & V Communications - (ARTIST) “DJ Lo-Skee”. All Parties have agreed by signature
to be bound by said terms and conditions listed below. L& V Communications.that being
the sole proprietor, Khalid Ali and or his designee that being Vivian R. Maney Ali, are
the exclusive authorized representatives of L..& V Communications None To Follow
This contract is subject to the following conditions:
1. Authorized contract and promoter agents: The client agrees to pay the full sum
of _____________________ Dollars ($ ____.00) to L.& V Communications - (ARTIST)
“ Dj Lo-Skee” and /or his designee Vivian R. Maney Ali, upon the arrival of the below
engagement in the payment forms of Cash, check, money order, or cashiers check made
payable to Khalid Ali. L.& V Communications does not guarantee exact arrival time.
No Partial payments accepted In the event of non-payment, L.& V Communications
retains the right to attempt collection through the Kentucky court system and/or
collection agency. Client will be responsible for all court cost, attorney fees to include
collection cost incurred by L..& V Communications The client will be billed separately
and assessed a $20.00 pre day assessment for the past due outstanding unpaid
balance until paid. It is mutually understood that a $35.00 non-sufficient fund fee will be
billed separately to the client for any returned checks.
2. Deposit: The Client agrees to pay L& V Communications. - DJ Lo-Skee - a $100
non-refundable retainer. Artist Is not confirmed for the event until this signed
agreement and outlined deposit has been received or waived. This contract must be
signed and dated by the client and artist. The required retainer must be attached and
mailed back within (5) days of the scheduled event. If This is not signed and dated by
the parties and mailed back with the required retainer within five (5) days, this contract
is Void. Until The Artist receives a valid (signed) contract and the required deposit if
applicable, at no time can the Artist name be used advertisement or promotions.
3. Cancellation and Peace Of Mind Substitution. It is mutually understood by all
parties if the client decides to cancel this contract, the client agrees to inform L.&V
Communications - (Artist) “DJ Lo-Skee” also Known as Khalid Ali of the cancellation
in writing via email at LandVComm@gmail.com overnight delivery or by direct telephone
voice contact at (270) 748-7722 between client and artist. The client will NOT received
a refund of any monies paid in advance (less the $100 retainer- if applicable)
UNLESS written cancellation is received three (3)days scheduled engagement. The client
agrees to pay 50% of gross price agreed upon if cancellation is received two (2) days
prior to the engagement . The client agrees to pay 75% of the gross price agreed upon
if cancellation is received less than two (2) days prior to the scheduled engagement
